Bilingual Care Planner

Location: Long Beach, CA
Salary: $26.00 USD Hourly - $31.00 USD Hourly
Description: Our client is currently seeking a Bilingual Care Planner
Field/remote based role (travel throughout LA County)
Must be bilingual in English & Korean

Role: Provide long-term care management to frail, economically disadvantaged, culturally diverse older adults, disabled adults, and caregivers to help them remain safely at home and prevent premature institutionalization.

Responsibilities:

Conduct telephonic assessments, care planning, and client monitoring.

Coordinate linkages with community-based services and resources.

Maintain legal and clinical records by completing necessary documentation timely.

Ensure timely implementation of services.

Develop and implement comprehensive care plans specific to client's goals.

Maintain ongoing documentation of contacts with clients and their support systems.

Communicate effectively with other disciplines working with clients.

Maintain accurate service records and follow contract guidelines for expenditures.

Stay current on Medicare and Medi-Cal policies, legislation, and community resources.

Participate in telephonic activities to maintain census and provide customer service.

Engage in Case Conference, Quality Assurance reviews, and other audits.

Perform other duties as assigned.

Qualifications:

Bachelor's Degree in Social Work, Psychology, Gerontology, or related field required; MSW preferred.

1-2 years of experience working with the elderly.

1+ year of experience in a hospital, home health agency, or related setting.

Knowledge of environmental concerns and developmental needs of older adults.

Strong organizational, verbal, and written communication skills.

Demonstrated customer focus and ability to work well with others.

Experience in case management or knowledge of case management techniques preferred.

Bilingual in English/Korean (proficiency test required).

Travel 50+% of the time within Los Angeles County; valid driver's license, automobile insurance, and reliable transportation required.
