Job Summary:
The Care Coordination Assistant supports the UM Nurse, Social Workers and Complex Care RN with non-licensee type task and functions within the department to ensure patient throughput remains timely. Is
responsible for selected functions related to Advance Directives, Utilization Review and Disposition Planning. The Care Coordination Assistant's responsibility is to support the Care Coordinators and patients requesting assistance with advance directives, DME, Medication pick up, delivery of MOON (Medicare Outpatient Observation Notification), transition of care items, delivery of IMFM (Important Message from Medicare), and other duties as assigned by supervisor. Is responsible for communication with the patient/family and entire care team on a timely basis.
Core Responsibilities and Essential Functions:
2. Disposition Planning
a. Supports the SW and UM nurse, Complex Care RN with assigned functions to prevent barriers for timely disposition gathers information and coordinates with care coordinator.
b. Provides resource information in a timely and efficient manner.
c. Collaborates with other members of the health care team to lend support.
d. Provides education to patient/family in understanding, letters being delivered and any follow up
e. Understands eligibility processes and criteria for both private and public local, state and federal resources.
f. Responds to referrals from hospital staff, physician offices, community and family to provide resource information and education.
g. Coordinates financial needs assessment for patients in need of assistance for follow-up care throughout the continuum.
h. Provides follow-up for patients needing post-discharge assistance.
i. Allows for any cultural or religious beliefs in providing service and continuity of care.
j. Ensures timely delivery of DME and transportation services by ensuring follow up
1. ASSESSMENT
a. Monitors for compliance of Medicare/Medicaid regulations
b. Communicates with appropriate Care Coordinator for UR denials received. Track and tend data for PI
c. Allows for any cultural or religious beliefs in providing service and continuity of care.
3. Documentation
a. Communicate barriers for timely disposition based on needed resources for safe transitions
b. Obtains all the appropriate signatures from patients on MOON and IMFM letters
c. Support updates in PI tracking tools, White boards, avoidable day modules, etc.
4. Professional Development and Initiative
a. Completes all initial and ongoing professional competency assessment, required mandatory education, population specific education.
Performs other duties as assigned
Complies with all Wellstar Health System policies, standards of work, and code of conduct.
Required Minimum Education:
High school diploma Required or
LPN with graduation from an accredited school. Required
Required Minimum License(s) and Certification(s):
All certifications are required upon hire unless otherwise stated.
