Care Support Coordinator
Monday-Friday 8a-5p PST
Must be within 50 miles of Cerritos, CA

The Care Support Coordinator works collaboratively with CareMore network providers, extensivity, APCs, market ClinOps leaders, PCPs, APCs, and specialists to positively impact and improve population health management programs around STARS and HEDIS performance. This includes identifying preventative care gaps, basic medical record review, patient engagement, patient education, condition-specific intervention, care coordination, documentation, and administrative duties. The Care Support Coordinator impacts revenue driving and cost of care savings through the HEDIS program by closing health gaps through intervention/engagement, increasing preventative care compliance, early disease prevention and detection, lowering BDK, and with the ultimate goal of impacting overall morbidity.
How you will make an impact:
• Working from a list of active patients, making outbound calls to schedule various exams, diagnostic testing, and labs that the patient is overdue for.
• This role includes but is not limited to: eligibility research, medical record review, scheduling, and direct patient support.
• Escalate patient needs to the appropriate members of the care team.
• Demonstrate high-quality patient service at all times.
• Complete ongoing training and coaching programs for further skills and knowledge development.
• Work collaboratively with our Clinical teams and other associates within the National Patient Care Center.
• Accurately and concisely document patient feedback and special needs during each call in our computer systems, enabling all other departments' success.
• As tenure, familiarity, and comfort in the role increase, associates within the Care Support team can be afforded new opportunities with additional patient workloads.
Minimum Requirements:
• High school diploma/GED required, associate degree preferred or equivalent in education and work experience.
• Up to 1 year of previous experience in an automated customer service environment; or any combination of education and experience, which would provide an equivalent background.
Preferred Skills, Capabilities, and Experiences:
• Associate or Bachelor's Degree.
• Medical experience/Terminology
• Healthcare certification: MA, Phlebotomy, CNA, Certified Coder.
• Exceptional oral and written communication skills
• Patient care experience
• HEDIS experience
• Care Coordination
• Working knowledge of the healthcare industry is strongly preferred.
• Previous experience and success in a telephonic customer service call center
• Durable data entry skills and ability to navigate multiple computer screens
• Enjoys working in a team-based environment with active collaboration with remote associates to maximize success.
• Thrives in a fast-paced environment that relies on the ability to multi-task and balance multiple competing priorities yet is still able to balance that energy and drive with sensitivity and compassion.
• Maintains regular and punctual attendance
For candidates working in person or remotely in the below locations, the salary* range for this specific position is $16.04 to $28.87
