The Support for Trauma Affected Refugees (STAR) program provides holistic and integrated trauma assistance services to underserved refugees and other eligible newcomers, including case management, health, psychological, and social adjustment services. The goals of the STAR Program are the integration and successful achievement of sustained physical, social, emotional, and economic well-being of those underserved refugees and other newcomers eligible for ORR services whose experience of trauma is impeding their ability to function effectively at home, school, work, or in social settings.

The CWS Houston STAR program primarily serves two targeted populations, Cuban Entrants, and survivors of human trafficking. We are actively recruiting case managers who have lived experience as either a Cuban Entrant or a survivor of human trafficking to provide peer support services.

Purpose

The Case Manager-STAR will be responsible for conducting community outreach to businesses, schools, and neighborhoods in Harris County, screening individuals to determine program eligibility, providing comprehensive case management support to clients to facilitate improvement in overall well-being, and facilitating community-based psychosocial support groups.

Responsibilities

  • Conduct community outreach to educate community members about the STAR program and services
  • Conduct phone screening of potential clients to determine program eligibility
  • Conduct intake and needs assessment of all STAR clients to assessing for the client's functioning in regard to school/employment, family, social engagement, and basic needs.
  • Develop a 6 month Individual and Family Wellness Plan for every client, identifying support services that may assist clients in becoming self-sufficient, such as ESL classes, transportation, immigration services, childcare, school enrollment, etc.
  • Conduct in-person or virtual monthly visits with clients
Coordinate and collaborate with the client's mental health provider(s) to plan and adjust services as necessary
  • Facilitate psychoeducational ag support in stressful or urgent situations, and helping to ensure that the client's rights are respected
  • Document all contacts, services, and referrals in client case files and maintain case note logs.
  • Complete all required reports and documentation in a timely manner.
  • Attend Peer Support Core Training and Mental Health Peer Specialist Training
  • Attend additional trainings and conferences as requested by supervisor.
  • Keep informed about issues affecting refugees, immigrant, and survivors of trafficking using current information to better identify and serve clients.
  • Represent the program at meetings and speaking engagements relevant to the responsibilities of the position.
  • Perform all duties in a culturally and linguistically appropriate manner.
  • Undertake other duties as assigned.

Qualifications

Education:
  • Bachelor's Degree in social service field or equivalent work experience.

Experience:
  • At least three years of previous work experience in case management required, and interest in immigrant and refugee populations.
  • Lived experience as a survivor of human trafficking or as a Cuban Entrant and in recovery from a mental health condition preferred.

Skills
  • Fluency in English and Spanish required
  • Willingness to appropriately share relevant details of lived experience preferred
  • Ability to advocate and liaise with social service systems
  • Ability to establish trust and rapport with clients and community partners
  • Ability to remain calm and use sound judgment in high stress situations
  • Excellent problem-solving skills with the ability to multi-task, prioritize duties, and manage time effectively
  • Certified Peer Support Specialist preferred
  • Strong written and verbal communication skills
  • Computer literacy with proficiency in MS Office applications and familiarity with data-tracking systems.
  • Ability to work independently and as a team member in a fast-paced, cross-cultural environment
  • Demonstrated success working and communicating effectively across lines of difference with respect to race, gender, ethnicity, language, religion, and age
  • Ability to develop rapport and form a respectful relationship with clients focused on helping them gain needed skills while using a strengths-based perspective
  • Ability to follow strict guidelines of client confidentiality and safeguarding
  • Valid driver's license, access to reliable vehicle with current insurance

Special Requirements

  • Standard work hours with occasional evening or weekend assignment
  • Occasional travel may be required
  • Successfully pass background checks
