Scope of Position

Community Health Workers (CHW) will primarily be working out in the community with specific target populations. CHW's will work closely with CCHE staff, community organizations/groups, and other agencies to increase health knowledge especially around cancer and cancer prevention. To provide resources and interventions to improve access to age-appropriate cancer screenings.

Position Summary

The CHW is responsible for working in partnership to plan and implement programming and projects within the community. Serve as a liaison between CCHE staff, faith-based organizations, community groups, and target populations. Assist in the promoting existing community outreach programs, screening programs and other initiatives as directed by the Outreach Analyst 2.

Duties and Responsibilities

  • Responsible for establishing trusting relationships with communities, community partners and organizations.

  • Serves on boards, coalitions, and relevant professional groups to facilitate and maintain these relationships.

  • Participates in outreach and education events while providing widespread support and encouragement to receive cancer related screenings and services. Conducts one-on-one and small group cancer education sessions.

  • Helps facilitate community connections for OSU researchers with the assistance/guidance of the Sr. Director.

  • Assists patients with completing applications and registration forms for screening mammography, cervical cancer and other cancer screening programs including conducting eligibility determination, enrollment, and follow-up with uninsured patients.

  • Provides ongoing follow-up, basic motivational interviewing and goal setting with community members/clients.

  • Displays knowledge about community resources appropriate to needs of community members in the population.

  • Provides referrals for services to community agencies as appropriate.

  • Helps patients connect with transportation resources and gives appointment reminders in exceptional circumstances.

  • Refers to OSUCCC patient navigators as appropriate.

  • Responsible for providing consistent communication to the CCHE Program Manager to evaluate progress, ensuring that provided information, and reports clearly describe progress.

  • Collects and maintains data, records, and completes reports for the programs he/she works on.

  • Assists in the preparation of articles, proposals, reports and educational materials for publications.

Minimum Required Qualifications

Bachelor's degree or equivalent experience. 2 years of relevant experience required. 2-4 years of relevant experience preferred.

Target Salary Range: $47,200 - $62,050
