Schedule: Working 8 hour shifts between 7am-5:30pm. Required to be on site 1-2 times a week with flexibility to work remotely some days. Quarterly presence at all hospitals.
Job Summary:
The Diabetes Program Coordinator is responsible for the development, coordination, monitoring, and ongoing success of the diabetes stewardship program for Froedtert Health. The Diabetes Program Coordinator will perform surveillance for patients at high-risk of poor outcomes related to diabetes or patients with poor glucose control. This pharmacist coordinator will lead committees to further diabetes stewardship efforts across the Froedtert & the Medical College of Wisconsin system. In conjunction with Froedtert's mission focusing on innovative community-academic partnerships, excellence in care and service, and progressive medical research and education; the pharmacist assures that patient care is optimized through compliance with the department's quality improvement strategies and the Joint Commission standards. The individual in this position will report directly to a Pharmacy Director.
EXPERIENCE DESCRIPTION:
A minimum of 3 years of pharmacist experience in area of diabetes or drug policy is preferred. Bachelor's degree or PharmD degree from ACPE certified school of pharmacy is required.
EDUCATION DESCRIPTION:
Successful completion of an ASHP Pharmacy Practice or specialty residency is preferred.
SPECIAL SKILLS DESCRIPTION:
Leadership skills and active membership(s) in state and national pharmacy associations.
LICENSURE DESCRIPTION:Active Wisconsin Pharmacist license (or eligible for) is required.
