PURPOSE:

The Jewish Board's Community Behavioral Health treatment programs provide compassionate, high quality, evidence-based services to individuals and families in the communities we serve. Our staff use a culturally competent, person centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resilience of our communities.

POSITION OVERVIEW:

The Director of Centralized Intake is a member of the Business Operations leadership team and is primarily responsible for the development, implementation and oversight of our new Centralized Intake, Assignment and Discharge Team, to ensure clinic and CCBHC clients have quick and efficient access to care, that these clients have a prompt assignment or re-assignment to a therapist and that our inactive and chronic no-show clients are discharged as quickly as possible, to ensure the smooth flow of all clinic and CCBHC clients through the system.

KEY ESSENTIAL FUNCTIONS (List the functions that are essential to achieve the goals):

The Director of Centralized Intake is responsible for optimizing all client intake, assignment/re-assignment and discharge activities in clinics and CCBHCs, and the supervision, oversight and development of the Centralized Intake, Assignment and Discharge Team.

Responsibilities:
  • Operationalize the Central Intake, Assignment and Discharge Team.
  • Devise ways to optimize intake, assignment/re-assignment and discharge processes.
  • Work in collaboration with Clinics, the CCBHCs and One Call to ensure a streamlined intake process for all clients seeking services.
  • Create and implement a robust, organized tracking system to monitor all pre-admit clients, clients scheduled for intake and follow up therapist assignment, clients awaiting therapist re-assignment, inactive clients and chronic no-show clients.
  • Actively manage all clients on the tracker to ensure smooth movement through the system.
  • Continuously monitor clinic capacity for potential bottlenecks that adversely impact access to care including clinical hires and terminations.
  • Reviews trends in staff turnover and impact on intake and assignment process.
  • Actively monitor therapist and provider caseloads to identify changes in caseload capacity across the system. This includes managing inactive caseloads, performing administrative discharges and working with the clinical team to complete discharge summaries.
  • Responsible for the Utilization Review process for all clinics and administrative discharge of inactive cases.
  • Responsible for timely completion of discharge summaries.
  • Prepare periodic Key Progress Indicator reports to track metrics required for CCBHC reporting.
    • Collaborate with program leadership on quality improvement initiatives
    • Collaborate with clinic directors to ensure the smooth operation of the Centralized Intake, Assignment and Discharge functions.
    • Collaborate with the Learning Department to assess ongoing training needs for staff around clinical care and engagement, including the incorporation of Evidenced Based Practices
    • Review team quality performance measurements and provide input for coaching performance/action plans as applicable.
    • Ensures that all complaints are resolved in a timely manner.

    Personnel:

    Responsible for ensuring appropriate staffing to support clinical care and evaluating staff performance. Duties include the following:

    • Timely completion of performance appraisals and appropriately addressing performance concerns

    • Conducting or participating in staff training, evidenced based treatment models consultation and development.

    • Monitor staff attendance and vacation scheduling to always ensure adequate coverage.

    Administrative Functions and Program Operations:

    As part of the leadership team, the Director for Centralized Intake will collaborate with clinics, One Call and CCBHC leadership in ensuring intakes, assignments and discharges are conducted efficiently; that these functions remain in compliance with relevant regulatory entities and is fiscally viable. Duties are as follows:

    • Facilitate team meetings, case conferences, and other meetings as assigned

    • Participate in clinic and / or CCBHC meetings as needed.

    • Participate in quality improvement and assurance activities, including completion of surveys and evaluations.

    • Liaise and collaborate with the Clinic Directors, One Call on a regular basis, and other departments needed.

    CORE COMPETENCIES for the position include:

    • Experience managing leaders/supervisors and staff.
    • Previous experience in the healthcare field preferred.
    • Knowledge of key performance indicators and reading data reports.
    • Pleasant, friendly attitude, with an ability to adapt to change.
    • Desire for personal/professional growth and development.
    • Superior problem-solving skills.

    EDUCATIONAL / TRAINING REQUIRED (List all that is required to achieve this position):
    • Master's degree in human services or business and New York State clinical licensure in social work (LCSW) preferred but not required.

    EXPERIENCE REQUIRED / LANGUAGE PREFERENCE (List what the desire to reach goals):
    • 3 to 5 years' experience in an operational leadership role in a clinical setting.

    COMPUTER SKILLS REQUIRED (List the computer skills needed):
    • Experience working with electronic medical records and proficient with Microsoft Office.

    VIVISUAL AND MANUAL DEXIERITY:
    • The candidate should be able to read documents for analytical purposes such as computer information, software, computer graphic design programs, etc.
    • Limited applications of manual dexterity and hand-eye coordination.

    WORK ENVIRONMENT / PHYSICAL EFFORT:
    • Hybrid work setting. Position entails a combination of in person and remote work.
    • Travel required between program locations and other Jewish Board sites.
