We are partnering with a local DME and Home Healthcare organization that is looking to add a Care Coordinator to their team!

Care Coordinator (Home Health Division)

Pay: $18.00-$21.00

Monday-Friday 8:00 AM-5:00 PM

Responsibilities:

• Review medical notes and charts
• reach out to nursing agencies to staff a caregiver for patient
• Provide updates to patients, nurses and caregivers
• Troubleshoot order issues
• Handle client/patient inquiries

Qualifications

• Strong client care mindset
• Attention to detail
• Home health experience a plus
