Job Title: Hospice RN Case Manager

Duties & Responsibilities: Hospice RN Case Manager sought for 13 week assignment, with the potential to extend. Schedule would be 5x8 days, Monday-Friday, with no on call or weekends required. Mileage is reimbursed at $0.50 per mile.

Minimum Requirements:

• SC or Compact RN license
• 1+ year hospice in home settings experience within the past 3 years
• Must have experience with case managing
• HCHB experience required
• BLS
