As a Mental & Behavioral Health Institute (MBHI) Systems Navigator, you will play a critical role in ensuring children and families receive timely and appropriate mental health services through effective coordination and communication. You'll work closely with internal care teams, community-based providers, and caregivers to help families navigate the mental health system and overcome barriers to care.

Key Responsibilities:

System Interaction

  • Manage referrals, schedule appointments, and coordinate care across internal departments and external mental health providers.

  • Act as a liaison, building relationships with service providers to ensure smooth transitions for patients and families.

  • Document interactions and care coordination in the Electronic Health Record (EHR).

  • Facilitate communication between care teams and external providers, ensuring continuity of care.

Family Liaison

  • Help families overcome barriers to accessing care, such as transportation, insurance, and system navigation.

  • Provide clear communication to caregivers about next steps, logistics, and what to expect in their care journey.

  • Elevate family concerns to the appropriate care team members, providing psychoeducation on the mental health system when necessary.

Collaboration with ED

  • Assist patients referred by PIRC Social Workers by scheduling follow-up appointments with appropriate providers.

  • Address insurance, location, and eligibility needs for services.

  • Follow up with families on referrals and help resolve barriers preventing access to care.

  • Educate families about crisis support and ensure they understand their care plan.

Discharge Planning

  • Work with the care team to coordinate outpatient care post-discharge.

  • Schedule follow-up appointments, manage referrals, and help families navigate any obstacles in accessing services.

  • Act as the point of contact for families between discharge and outpatient care, addressing any crises that arise.

Care Coordination

  • Coordinate with school services teams for students returning to school after hospitalization or intensive treatment.

  • Facilitate transitions from pediatric to adult mental health services, attending appointments and ensuring a smooth hand-off to new providers.

  • Serve as a resource for private pay patients, assisting with navigating the system.

  • Manage care coordination for patients with multiple services (e.g., Psych and DDBP), ensuring appointments are aligned, and concerns are addressed.

Qualifications:

- Bachelor's degree in Social Work, Psychology, or a related field.

- Experience working in healthcare or mental health services, with knowledge of care coordination and patient advocacy.

- Strong organizational and communication skills.

- Ability to navigate complex systems and build relationships with internal and external stakeholders.
