Description:
Our client is seeking Nurse Practitioners to conduct in-home health assessments across the USA.
Key Responsibilities:
*Conduct comprehensive health assessments
*Review medical history and medication adherence
*Educate patients and collaborate with healthcare teams
*Maintain accurate documentation
Requirements:
*Master's degree and NP state licensure and an active Board Certification AANP OR ANCC
*Strong clinical and communication skills
*Ability to travel ( Going to various Homes to conduct assessments in your assigned County)
